How It Works
Conducting a risk assessment for pre-diabetes or diabetes is the easiest way to build awareness among your Lions club or target audience. It’s a simple way to screen for possible diabetes risk, and to prompt folks to seek follow-up care.
Multiple risk assessment forms are sometimes available from the American Diabetes Association offices, or they may be downloaded and printed.
This site offers two forms: one for pre-diabetes risk, and another for diabetes. They are essentially the same – both check for the risk of elevated blood sugar (hyperglycemia). Select a form below to download it.

Step One: Getting Started
Select and obtain a sufficient number of risk assessment forms for your event. If you download and print them, use a color printer if available. Advertise the event in advance to your target audience.

Step Two: Execution
Conduct the event. You may wish to add educational materials, e.g. diabetes flyers, a PowerPointâ„¢ presentation, a featured speaker or program, etc. See the available educational resources on this site.

Step Three: Completion
For privacy, we recommend that you do not collect the forms or encourage people to share their results. Recommend that those who are identified at risk see follow-up care available in your area.
